Autumn Home Care team is a natural complement to your care team. We’ll work with your existing Palliative/Hospice team to provide a flexible solution that works with the needs of you or your loved one.
Our Caregivers are available 24 hours a day – 7 days a week. We can help for a few hours each week or support your palliative team with ‘round the clock care.
We can step in to maintain a comfortable, high-quality home life before, during, and after treatment and will customize your care plan to your specific needs. While your palliative care team is charged with keeping you or your loved one comfortable, we make life even easier by providing home care support services, such as personal care, companion care, household support, and more.
Our team will coordinate with the Palliative/Hospice care team so that you or your loved one is getting the top-quality care and follow-up support needed for at-home living.
We also gladly provide senior respite care to family caregivers so that they can get the rest and support they need to stay healthy, too.

ALS & Parkinson
More and more ALS patients are opting for care in the comfortable surroundings of their home. This is especially the case for the elderly who are often too frail to be moved successfully.
